Composition

Active ingredients: 1 tablet contains albendazole 400 mg;

Excipients:

  • corn starch,

  • gelatin,
  • methylparaben (E 218),

  • colloidal anhydrous silicon dioxide,
  • sodium starch glycolate,
  • magnesium stearate;
  • mixture for film coating Opadry II White (hydroxypropyl methylcellulose, titanium dioxide (E 171), lactose monohydrate, polyethylene glycol, triacetin).

pharmachologic effect

Aldazol is a broad-spectrum anthelmintic agent; also has activity against pathogenic protozoa. Acts on intestinal and tissue forms of parasites; active against eggs, larvae and adult helminths.

The mechanism of action is due to the selective suppression of beta-tubulin polymerization, destruction of cytoplasmic microtubules of cells of the intestinal tract of helminths; suppression of glucose utilization, blocking the movement of secretory granules and other organelles in the muscle cells of roundworms, which causes their death.

The active ingredient of the drug is active:

against intestinal parasites, including nematodes - Ascaris lumbricoides, Trihiurus trihiura, Enterobius vermicularis, Ancylostoma duodenale, Necator americanus, Strongiloides stercoralis, Cutaneous Larva Migrans; cestodes - Hymenolepsis papa (dwarf tapeworm), Taenia solium (pork tapeworm), Taenia saginata (bovine tapeworm); trematodes - Opisthorhis viverrini, Clonorchis sinensis; protozoa - Giardia lamblia (intestinalis and duodenalis)

  • against tissue parasites, including cystic echinococcosis (Echinococcus granulosus) and alveolar echinococcosis (Echinococcus multilocularis). The drug destroys cysts or significantly reduces their size in patients with granular echinococcosis. With cysts caused by Echinococcus multilocularis, the majority of patients experience an improvement or stabilization of the disease, and some have a complete cure.
  • in the treatment of neurocysticercosis caused by the invasion of larvae of Taenia solium (pork tapeworm), capillariasis caused by Capillaria philippinensis.

Albendazole is poorly absorbed from the gastrointestinal tract (less than 5%); unchanged in blood plasma is not found. Oral bioavailability is low (about 30%). The simultaneous intake of fatty foods approximately 5 times increases the absorption of albendazole.

It is rapidly metabolized in the liver during the "first pass". The primary metabolite, albendazole sulfoxide, has half the pharmacological activity of the parent substance.

The maximum concentration of albendazole sulfoxide in the blood is reached 2 to 5 hours after application; connection with plasma proteins - 70%. Albendazole sulfoxide is almost completely distributed in the body; found in urine, bile, liver, in the wall of the cyst and cystic fluid, cerebrospinal fluid.

Albendazole sulfoxide in the liver is metabolized to albendazole sulfone (a secondary metabolite) and other oxidized products devoid of pharmacological activity. The half-life of albendazole sulfoxide is 8-12 hours. It is excreted in the form of various metabolites mainly by the kidneys, as well as with bile.

After prolonged use of the drug in high doses, its elimination from cysts continues for several weeks.

In case of impaired renal function, the total clearance of albendazole and its main metabolite does not change.

When liver function is impaired, the bioavailability of albendazole increases; the maximum concentration of albendazole sulfoxide in the blood doubles, and its half-life is lengthened.

Indications and dosage

Aldazole is used for the following diseases: enterobiasis, ascariasis, necatorosis, ankylostomiasis, trichinosis, clonorchiasis, opisthorchiasis, teniasis, neurocysticercosis (caused by cysticercus (larval form) of pork tapeworm), toxocariasis, giardiasis, strongyloidiasis.

Also, Aldazol is used for echinococcosis of the lungs, liver, peritoneum, provoked by the larval form of Echinococcus granulosus.

In addition, Aldazole may be prescribed in the presence of cutaneous larvae migrans.

Treatment with Aldazol does not require laxatives, adherence to a special diet, or the use of enemas.

Aldazol is used in adults and children over three years old. The drug is taken orally during meals, not chewed, washed down with water.

The dosage and treatment regimen with Aldazol depends on the type of disease. So, for ascariasis, ankylostomiasis, enterobiasis, necatorosis and trichinosis, Aldazol is used once in a dose of 400 mg (1 tablet). The course of therapy should be repeated after 3 weeks.

In the case of enterobiasis, it is recommended to simultaneously treat all persons living together.

For clonorchiasis and opisthorchiasis, Aldazol is used at a dosage of 400 mg 2 times / day for 3 days.

Overdose

In case of an overdose of the anthelmintic drug Aldazol, the following symptoms are noted: visual impairment, nausea, sleep disturbance, vomiting, speech impairment, visual hallucinations, indigestion, fatigue, dizziness, tremor, enlarged liver, tachycardia, jaundice, loss of consciousness. There may also be an orange or brownish-red coloration of stool, urine, saliva, sweat, skin, and tears, in proportion to the dose received.

Treatment includes drug withdrawal, gastric lavage, symptomatic therapy.

Side effects

When using the drug Aldazol, the following side effects may occur: stomatitis, nausea, dyspepsia, dry mouth, vomiting, abdominal pain, flatulence, diarrhea, constipation, jaundice, liver dysfunction, insomnia or drowsiness, headache, confusion, dizziness, disorientation, convulsions, hallucinations, decreased visual acuity, agitation, fatigue, pancytopenia, leukopenia, granulocytopenia, thrombocytopenia, agranulocytosis, pruritus, urticaria, skin rashes, pemphigus, fever, dermatitis, angioedema and / or pain in the muscles insufficiency, inflammation of the joints, photosensitization, menstrual irregularities, increased blood pressure, hyperthermia, reversible alopecia.

Aldazole is contraindicated in case of hypersensitivity to the ingredients of this drug. Aldazole is not used for retinal damage, during pregnancy, as well as before a planned pregnancy during a period lasting one menstrual cycle.

Aldazole is not used in children under three years of age.

In case of suppression of bone marrow hematopoiesis, cirrhosis of the liver, liver failure, as well as cysticercosis involving the retina of the eye, Aldazol can be used with caution.

In case of neurocysticercosis with eye damage, an examination of the retina of the eye is necessary before starting treatment because of the risk of aggravating its pathology. With neurocysticercosis, appropriate therapy with glucocorticoid agents (to prevent a hypertensive attack in the first week of anticystic therapy) and anticonvulsants should be carried out.

During drug therapy, it is necessary to monitor the activity of hepatic transaminases at the beginning of each course of treatment; if the upper limit of the norm of enzyme activity is 2 times exceeded, the use of the drug must be suspended until they are completely normalized.

At the beginning of treatment and every 2 weeks of each 28-day cycle of drug use, it is necessary to control the number of leukocytes. With the development of severe leukopenia, it is necessary to discontinue drug therapy; treatment can be continued if the degree of decrease in the number of leukocytes is insignificant and leukopenia does not progress.

Patients receiving high doses of the drug should be closely monitored, with constant monitoring of blood and liver function.

This dosage form of the drug is not intended for use in children under 3 years of age. During the period of treatment, one should refrain from driving and performing work that requires increased attention and reaction speed.

If the symptoms of the disease do not begin to disappear, or, on the contrary, the health condition has deteriorated, or undesirable phenomena have appeared, consult your doctor for advice on further use of the drug.

Dexamethasone, praziquantel and cimetidine increase the level of albendazole metabolites in the blood.

Albendazole is an inducer of cytochrome P450 microsomal enzymes. Aldazole accelerates the metabolism of many drugs.
